Instruction to fill the form

These instruction are for all the columns in the form.

  1. Leave the boxes of "FORM NUMBER" and "PLACEMENT" blank. These are for office use only.
  2. Tick in proper Box of Male or Female.
  3. In the next series of boxes, you provide information about your performance of last examination.
  • Roll No:
    Write the Roll number of your last examination.
  • Year:
    Write the year of your last passing examination.Remember that only those are eligible who passed their examination in 2000 Annual or 1999 Supplementary Examination.
  • Marks Obtained:
    Write the total marksyou obtained in your last examination. O'Level students should leave this bos blank.
  • Total Marks:
    Write the total marks of your last examination. O'Level students should leave this box blank.
  • Grade:
    Write your grade achieved in your last examination. O'Level Students should leave this block blank.
  • Group:
    Tick one of the perspective boxes.
  • Examination:
    Tick one of the boxes of "Annual" or "Supplementary".
  1. Write the name of the school you last attended.
  2. Write the district in which your school is located. There are five districts in Karachi:East,West,Central,South and Malir. This Information is to be provided by only those students who passed their last examination from Karachi.
  3. Tick in the box of the board from which you took your last examination.
  4. Write your name in BLOCK LETTERS.
  5. Write your father's name in BLOCK LETTERS.
  6. Write the National Identity Card number of your father or guardian.
  7. Write your present address. Make sure that it is correct so that you can be contacted if your form has any mistake.
  8. Wirte your telephone numbers. Those of you who do not have a telephone at your home or office should try to give a number on which a message can be left, so that you can be contacted in case there is a mistake in your form.
  9. You must be very careful in setting your priority of colleges and filling in the CHOICE OF COLLEGES/HIGHER SECONDARY SCHOOLS column.Check the "detailed information about Colleges and Higher Scondary Schools" and keep in mind the following factors to avoid losing all your choices:
  • The number of seats allocated according to the capacity of the colleges.
  • The percentage on which the admissions were closed last year. Compare it with your percentage before choosing a college.
  • The public transport facility available for the area in which a college is located. Make sure that the college of your choice is easy to reach for you and you are not late for classes or prone to absenteeism, because the college authorities will cancel your admission on grounds of violating the rule of punctuality and regularity.
  • The faculties and subjects combination available. Humanities students should be particularly careful in this regard.

The placement process will be computerizedm, so complete this column and give all eight choices. These priorities will not be changed once you have submitted the form and once you are placed in a college, you will not be given any option. If your marks do not allow your placement in any of the eight colleges of your choice, the computer will automatically place you in a college according to the district of your school. Remember NOT to fill in the code numbers. You do not know them. The office will fill them in.

  1. Tick any one of your faculty choice. Check the list of faculties and keep in mind the restrictions of the boards of examinations. For example, those who passed their last examination from "Genaral Group" cannot choose Science Pre-Engineering, Pre-Medical and Science General Group. The students who passed their last examination from Technical Board can not choose Pre-Medical group. The faculty of Home Economics is only meant for female candidates.
  2. You can not apply for admission in two faculties using one form. To apply in another faculty,fill another form.
  3. Get the signature of your father/guardian.
  4. Put your own signature.
  5. Students are not required to fill the receipt portion of the form.
  6. Keep the returned portion of the form safely. This is the proof of your submitting the form and will be required for checking your placement and getting permission to enter the college where you get the placement.

Admission of O'Level student shall be confirmed when they will submit their results.

Attachments
  • You are required to attach three Powder Copies of your Placement Form.
  • You are required to attach an attested copy of your marks-sheet.
  • O'Level students should attach the attested copy of their MOCK EXAM RESULTS and a GRADE SURETY CERTIFICATE from your school.
